Do we select you as a care team?
Given our limited capacity and the desire of our experts to spend sufficient time on each trajectory, only a few care teams can participate. We will make a selection and assess together whether you meet the subsidy requirements. Our partner NEWID will then draw up a plan of action for joint submission, in consultation with you and our experts.
